3. Prescription Policies
3.1 Prescription Requirements

3.2 Refill Policies

3.3 Unclaimed Prescriptions
Prescriptions that are not picked up within 14 days may be returned to stock. You will be notified before this occurs. Controlled substances are subject to different timelines as required by law.
4. Payment Terms
Payment is due at the time of service unless other arrangements have been made. We accept cash, credit cards, debit cards, and most insurance plans. You agree to:
- Pay all applicable copays, deductibles, and coinsurance amounts
- Be responsible for the full cost if insurance coverage is denied or unavailable
- Provide accurate insurance information and notify us of any changes
- Pay any balance due within 30 days of billing

9. Termination
